New Patients: Blue

Fill out Walk In/Phone Intake Form on Desktop
  • https://bodysolutions.infusionsoft.com/app/form/cfd4ecf69246ef0d59f974075ac57580

    • Fill out Credit Card Form on Desktop (for Cancellation Policy)
    • On Practice Suite Schedule:
      • Find a FULL 60 min open block & click on it to schedule
      • Check the New Patient box on the scheduling tab (top left)
        • Input all New Patient info on the right hand side
        • On the left side:
          • Reason: Evaluation
          • Schedule Status: Pending Verification
          • Schedule Type: New Patient
          • Appt. Duration: 60 Min
          • Provider: Dr. Steve
          • SAVE

Returning Patients: Blue

  • Search for patient account on system:
    • Search tab (left side menu)
    • Input Last Name
    • Verify patient info & Update if necessary:
      • Address
      • Phone Numbers
      • Email
      • Insurance (if different, then just write down new insurance info: ID #, Group #, Subscriber, Provider Services number on the back)
      • If all information is the same, fill out Returning Patient Intake on desktop
      • If some info has been updated, fill out New Patient Intake Form on desktop
  • On Practice Suite Schedule:
    • Click empty 60 min slot
    • Search for patient by last name
    • Reason: Evaluation
    • Schedule Status: Pending Verification
    • Schedule Type: New Patient
    • Appt. Duration: 60 min
    • Provider: Dr. Steve
